Year one · full medical partnership
Optimization Program
$3,333 per year

The path for long-term support after your Reveal Assessment, for patients who want full partnership, monitoring, and prescription management.
What's included
- Four visits per year with both providers, in person or virtual (in-person visits are required every other appointment)
- One-month telehealth check-in after your initial assessment
- Prescription management, including bioidentical estradiol, progesterone, testosterone, and thyroid medication
- Lab monitoring for prescriptions, plus one follow-up lab panel per year
- Four 30-minute nurse coaching sessions per year, one per quarter
- Portal messaging for questions between visits
- Your Fuel Plan and Movement Plan, built at your assessment and revisited at six months
- Specialty testing at our cost, with results review included
- Discounted imaging and supplements through Fullscript at member pricing
- Body composition at every visit and invitations to wellness education workshops

Year one is intensive. Once your foundation is built, most people need steady oversight rather than constant recalibration.

- Two assessments per year, every six months
- Prescription management, including bioidentical hormones and thyroid
- Two 30-minute nurse coaching sessions per year, one every six months
- Portal messaging for questions between visits
- Specialty testing at our cost, with results review included
- Supplements through Fullscript at member pricing

Wellness Consulting

One hour, in person, with both providers
An annual check-in for the self-directed: one visit a year with both of us, for people who take their plan and run with it on their own.
What's included
- One-hour visit with Dr. Mark and Tandi
- Interpretation of your labs
- Education about metabolism, hormones, and lifestyle patterns
- Recommendations you can apply independently or discuss with your PCP
This is a good fit if
- You want expert insight or a second opinion on your labs
- You're looking for education and direction without medications
- You prefer to take information back to your primary care provider
- You're exploring functional medicine but aren't ready for ongoing care
Lab work is not included. Anything we order is billed at our discounted self-pay rates. This visit does not include prescription management, hormone or thyroid adjustments, required follow-ups, or establishment of medical care.
Available on its own or alongside any path
Holistic Coaching with Tandi
$333 intake · $222 per session
$1,111 for a six-session package
A whole-person practice with a Board-Certified Nurse Coach, Reiki practitioner, yoga teacher, and vocal toning and frequency practitioner. A 90-minute intake, then 50-minute sessions at your pace.

- 50-minute sessions · $222 each, weekly, every other week, or monthly
- Six-session package · $1,111
- Nurse coaching across nutrition, movement, sleep, stress, and mindset
- Habit design tailored to your life
- Breathwork, sound healing, vocal toning, Reiki, or cacao ceremony as appropriate

This isn't about rejecting traditional care. Paying directly lets us focus on prevention, optimization, longer visits, comprehensive hormone and metabolic mapping, and lifestyle-based root-cause care.
Labs, HSA, and FSA
Our national lab partnerships let us offer discounted self-pay lab rates and transparent pricing. We always use recent labs from your PCP when applicable, so we avoid unnecessary duplication.
HSA and FSA funds can be used in most cases for our services, lab testing, imaging, and some supplements with clinician notes.
